Sports Physical - REQUIRED
Documentation of a current physical must be on file in the school office prior to a student participating in any athletic tryouts, practices, or games. Click this link for the accepted IHSAA physical form- Physical Form. Pages 1 & 2 are filled out by the physician conducting the physical. Return the signed physical examination form to the school office before the first sport is played each year. Valid physicals must be dated after April 1 of the previous spring.

Athletic Packet - REQUIRED
The following forms are combined in the online packet:

Concussion/Sudden Cardiac Arrest 
This annual training is required by all student-athletes and their parents/guardians before participating in their first sport of the year.  Please review the concussion and sudden cardiac arrest information included in the online packet.  

CYO Consent Form
Each school year, a CYO Consent Form must be completed (signatures required by student and parent) before participating in CYO sports. This form must only be completed once annually, even for a student who plays multiple sports. CYO sports include Volleyball, Football, Cheerleading, Basketball, Soccer, Softball, and Track.

Emergency Medical Consent and Contact
Documentation for emergency medical treatment consent as well as emergency contacts must be submitted as part of the athletics packet. This information is kept with the coaches during all athletic activities in the event of an emergency.

Coaching and Athletic Board Volunteer Forms
All persons interested in volunteering as either a member of the Saint Charles Athletic Board or as a Coach are asked to complete and turn in a Letter of Interest form.  Click the links below to access the appropriate form. Print, complete, and turn in to the school office (Attn: Athletic Board) or to a member of the Athletic Board.
